Are you caring for someone who is experiencing memory loss and is no longer safe to be alone in their home?  Are you a caregiver who is also working full-time or simply need a break from day-to-day-care?  If you are struggling to find a solution - your answer just may be Hebrew Adult Day Center.
 
Our vision is to provide the highest quality compassionate care to individuals experiencing memory loss while providing essential support to those people who are providing caregiving duties all day long. Hebrew Adult Day Center is a medical-model and is staffed by attentive and caring health care professionals who provide health care supervision, engaging therapeutic activities, nutritious meals, and field trips that enhance participants' quality of life and encourage social engagement.  
 
The Adult Day Center has a dedicated space located at 1 Abrahms Boulevard in West Hartford, Connecticut.  Our main room features a full kitchen where clients can bake, a separate greenhouse where clients care for plants, and an outdoor patio where clients garden, enjoy leisurely walks and simply bask in the sunshine with each other!
 
Benefits of Enrolling in an Adult Day Program 
 
  • Enhancing Quality of Life:  Hebrew Adult Day Center provides participants with a sense of purpose and routine, which can significantly improve their overall quality of life.
  • Relieving Caregiver Stress:  Caring for someone with memory impairment can be very demanding and extremely stressful.  Adult day services offer caregivers much-needed respite, allowing them to attend to their personal needs knowing their loved one is in a safe, supportive environment.
  • Promoting Health and Wellness: Access to health services in the Adult Day Center ensures that participants receive regular medical attention if required, daily medication management, nutrition counseling, assistance with showers and bathing, and daily physical activities to promote physical fitness.
  • Fostering Social Connections: Loneliness poses the same health risks as smoking 15 cigarettes a day.  Day services for adults with cognitive impairment provide a place for participants to build friendships and engage in social and physical group activities which are critically important for mental health and emotional well-being.
 
 
Participants can attend from one to five days a week, for full or half (4 hours) day. 
Transportation available within a 7-mile radius for a minimum fee.

Candice Smith, BS, LPN
 

Ms. Smith joined Hebrew Senior Care in 2019.  Ms. Smith has a bachelor's degree in healthcare administration in addition to being an LPN.  Ms. Smith has more than 12 years of nursing experience and develops care plans, performs assessments, and educates families on specific health care issues.  Ms. Smith has been a charge nurse on an alzheimer's unit and has a specific interest in working with and caring for individuals with cognitive impairment.  Ms. Smith is dedicated to her clients and their families and believes in providing patient-centered care.  Ms. Smith loves her work, adores her clients, and will do whatever is needed to help her families. For more information or to schedule a tour or a ½ day trial please contact Candice Smith at 860.523.3857 or email at csmith@hebrewseniorcare.org.
